An engraved medical ID bracelet or necklace presenting a concise overview of your condition, allergies, and medicines will alert a doctor or medic before starting treatment. This can ensure appropriate and timely medical care, particularly when you might not be able to speak for yourself in a medical emergency. Medical bracelets are available through the National Kidney Foundation of Northern California for $8.50 or $11.00, depending on how many sides are engraved.

To order a bracelet or necklace, please download the application listed below and include it with a check or money order, made out to the National Kidney Foundation of Northern California, and mail to NKF of Northern California 131 Steuart St. #520, San Francisco, CA 94105, Attention: Yuriko Kimura.

If you are a kidney patient in need of Medical Identification Jewelry, but are unable to afford the cost, please contact the social worker at your dialysis or transplant unit in regards to this program. Applications will only be processed through your healthcare professional. Orders are generally filled within three weeks, and will be delivered to the patient or social worker, depending on who placed the order.
